The Titan submersible implosion two years ago that caused the deaths of five people was "preventable," according to a Coast Guard investigation report released Tuesday.

The Coast Guard released a 300-page report of the Titan implosion that occurred in June 2023, when the submersible lost communication with its OceanGate handlers, NBC News reported. Days after communication was lost and the search for the Titan off the Newfoundland coast began, officials said that it was determined the submersible imploded and instantly killed all five people on board.

Coast Guard investigators said Tuesday that there were "multiple contributing factors" that led to the entirely "preventable" implosion of the Titan.

“This marine casualty and the loss of five lives was preventable,” said Jason Neubauer, chairman of the U.S. Coast Guard Marine Board of Investigation charged with investigating the Titan disaster.

"The two-year investigation has identified multiple contributing factors that led to this tragedy, providing valuable lessons learned to prevent a future occurrence. There is a need for stronger oversight and clear options for operators who are exploring new concepts outside of the existing regulatory framework."
